Primary Care

At North Shore Community Health, we deliver high quality, culturally inclusive, comprehensive primary care for adults, children, and adolescents. We also offer services such as health education, translation, and the arrangement of transportation to assist with and promotes access to health care.

North Shore Community Health offers discounted services to eligible patients and also provides services regardless of patients’ ability to pay.

North Shore Community Health offers patient-centered and integrated care that responds to your unique needs.  Services include: annual physical examinations, health screenings, school physicals, immunizations, sick visits, chronic disease management, and patient education. We also provide comprehensive women’s health care including confidential family planning and STD testing, cancer screening (mammography, cervical, and colorectal,) preconception care, and treatment and evaluation of women with complex gynecologic and menopausal issues.  NSCH also provides treatment for substance use disorders and HIV and Hepatitis C.

No one will be denied access to services due to inability to pay; and there is a discounted/sliding fee schedule available based on family size and income.
