Health Insurance Enrollment

We encourage anyone who is uninsured or who feels that they are paying too much for health insurance to make an appointment with our Patient Access Navigators so that we might help you to enroll in affordable health and dental insurance or to assist you in reducing the cost of your next visit.
Make an appointment to come to see one of our certified application counselors at Salem Family Health Center, Peabody Family Health Center, or Gloucester Family Health Center.

We offer a mix of telehealth and in-person services with our Patient Access Navigators. While telehealth appointments are available throughout the week, we also have Patient Access Navigators available at our different sites:

Salem:
Tuesday 10am – 4:30pm (Walk-In Hours from 1 pm – 4:30 pm)
Thursday 10am – 4:30pm (Walk-In Hours from 1 pm – 4:30 pm)

Peabody:
Wednesday 9am – 4pm
Friday 9am – 4pm (Walk-In Hours from 1-4pm)

Gloucester:
Wednesday  1pm – 4pm (Walk-in hours from 1-4pm)

Necessary Paperwork

To make your insurance enrollment process as easy as possible, please bring any of the following that may apply:
Proof of Identity & Citizenship Documents:

If you are 16 years old or younger, please bring one of the following: 

  • Birth Certificate or Any Passport, U.S. or Foreign
  • An affidavit signed under perjury by a parent or guardian attesting to the child’s identity
  • Statement of Physician or Midwife who was in attendance at the time of birth

 

If you are 16 years old or older please bring ONE of the following:

  • U.S. Issued Passport

OR

  • Birth Certificate AND ONE of the following:
  • Current MA License
  • School ID with a photograph of the Individual
  • US Military or Draft Record
  • Legal Permanent Resident Card
  • Student, Work or Tourist Visa

 


Proof of Income Documents:

Please bring ONE of the following:

  • 2 Consecutive Pay Stubs
  • Last Tax Return – schedule C if Self-Employed
  • Social Security, Retirement, or Unemployment Statement of Benefits document stating how much you get monthly.
For Non-US Citizens:

Please bring any paperwork you may have pertaining to immigration status. In Massachusetts, there are insurance options for all residency statuses, including undocumented non-citizens or those who are unlawfully present. Any documentation you have that can prove your current status will help with your application.

Patients can get help from the Health Safety Net if they are undocumented non-citizens/unlawfully present. Immigration status does not matter for this program. Undocumented non-citizens who meet the eligibility requirements can get HSN funds. Social Security number is not needed.

NSCH ensures that no one will be denied access to health services due to their inability to pay.

Sliding Fee Program

Our sliding fee program allows us to reduce or “slide” the fees for the medical and dental care of you and your family depending on your household size and annual income. You can apply for the program if you need assistance to help you pay for your care.
