What is a Federally Qualified Health Center?

North Shore Community Health is a Federally Qualified Health Center (FQHC) and is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employer under 42 U.S.C. 233(g)-(n). NSCH receives a grant under Section 330 of the Public Health Service (PHS) Act (42 United States Code Section 254a).

FQHCs provide comprehensive health services to underserved people. The first FQHC was actually in Boston, Massachusetts where services for moderate- and low-income people were severely limited. Because community members were the ones who organized to provide primary care to their own neighborhoods, they also comprised the majority of the board of directors. Today, FQHCs require at least 51% of the board to be a patient.

No one seeking care at any of our sites will be denied access to services due to inability to pay. We offer a discounted/sliding fee scale based on family size and income. More information can be found on our Health Insurance Enrollment page.

What is a Patient Centered Medical Home?

Patient-Centered Medical Homes (PCMH) are designed to ensure patients receive comprehensive and thorough care. This coordinated care is driven by an entire team of primary care providers, including physicians and nurse practitioners, nurses, medical assistants, community health workers, patient representatives and referral coordinators, hygienist, and providers from other departments as well.

Each of our sites deliver high quality, culturally competent, comprehensive primary care – medical, dental and behavioral health services. NSCH also offers supportive services such as health education, translation, health insurance eligibility and enrollment assistance, and transportation that promote access to health care.

Primary care providers (PCPs) are the drivers and coordinators of their patients’ health needs. This entails management of chronic issues, referrals to specialists, admissions, and follow ups for check-ups and tests. The PCMH model focuses on the patient’s needs and getting to know patients in long-term relationships. We want to provide the most exceptional care with your preferences and allow you to be directly involved in your care.
