Massachusetts Health Quality Partners Primary Care Services Survey

MHQP is a local measurement and reporting organization that provides date-driven insights to health care organizations, such as North Shore Community Health. NSCH patients may receive a survey in the mail from Massachusetts Health Quality Partners (MHQP). We are working together with Mass Health to gather feedback on the primary care services we provide. Please consider completing this survey to help us in continuing to provide excellent care to patients in our communities. Your participation is voluntary, but your answers really do make a difference.

Your responses tell us what works well and where improvements are needed. Survey results are confidential. If you receive a survey in the mail or via email, we would appreciate your taking the time to participate. The survey should take approximately 10 minutes to complete.
