About Us

North Shore Community Health has three full-time family practice sites: Salem Family Health Center, Peabody Family Health Center, and Gloucester Family Health. NSCH has two School-Based Health and Behavioral Health Centers: Salem High School Teen Health Center, Peabody Veterans War Memorial High School Teen Health Center and four behavioral health only sites in the following Salem schools:  Collins Middle School, Bates Elementary School, Bentley Academy Charter School, and Horace Mann Laboratory School.

Each site delivers high quality, culturally competent, comprehensive primary care – medical, dental and behavioral health services. NSCH also offers supportive services such as health education, translation, health insurance eligibility and enrollment assistance, and transportation that promote access to health care.

No one seeking care at any of our sites will be denied access to services due to inability to pay. We offer a discounted/sliding fee scale based on family size and income. Click here for the current sliding scale document.

North Shore Community Health is recognized by NCQA  and has been designated as PCMH PRIME Certified by the Health Policy Commission.

North Shore Community Health is a member of the Massachusetts League of Community Health Centers and the National Association of Community Health Centers.

North Shore Community Health is a Federally Qualified Health Center and is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employer under 42 U.S.C. 233(g)-(n).  NSCH receives a grant under Section 330 of the Public Health Service (PHS) Act (42 United States Code Section 254a).

Salem, Peabody and Gloucester Family Health have earned the National Committee for Quality Assurance’s Patient-Centered Medical Home (PCMH) Level Three Recognition (the highest level of recognition).

PCMH PRIME Certification is for practices in Massachusetts that demonstrate the ability to address behavioral health in primary care. The PCMH PRIME Certification Program is a collaboration between the Massachusetts Health Policy Commission (HPC) and National Committee for Quality Assurance (NCQA).
