Responsibilities:
- Communicate with members of patient’s care team outside of the health center as needed, which includes specialists or visiting nurses, to help with coordination of care.
- Serve as a liaison between NSCH and other care management programs and community-based organizations.
- In collaboration with CHWs, ensure patient access to local community resources and help link care teams/patients with the appropriate resources.
- Connect primary care team patients with needed behavioral health services, flexible services, Community Partners, respite care and other services.
- Guide care team in accessing durable medical equipment, health services, and other community-based social services for patients.
- Provide care coordination, facilitate care transitions, support the completion of referrals, and/or provide and confirm appropriate follow-up with care providers both within NSCH and with collaborating community care providers.
- Oversee hospital and emergency department discharge workflows for patients on assigned panel.
- Engage patients at their homes or other locations where the member may be located.
- Meet patients while they are in the hospital, when appropriate, to provide education and support around the discharge process and transition members into care management.
- Collaborate with specialty nursing care teams to connect patients with Hep C/HIV care, substance use disorder treatment, chronic disease/diabetes care, and anticoagulation management.
- Perform job duties of the PC Team RN as needed, which include:
- Collaborate and communicate with team primary care providers and other care team members to ensure quality and continuity of patient care on the primary care team
- Participate and coordinate team huddles
- Manage clinically related phone calls for the team including but not limited to: triage of same day appointments, abnormal labs, medication questions, and hospital discharges
- Review refill requests from pharmacy and patients based on medication refill protocol; Reconcile patient medication lists and attach pertinent information necessary for provider to authorize the refill
- Complete patient forms to the furthest extent possible, including prior authorization forms, request for VNA services, etc.
- Assess and evaluate patient care needs
- Analyze and interpret data and make informed judgments related to patient care.
- Evaluate outcomes of basic nursing interventions and initiate changes in care plans.
- Perform nursing interventions
- Administer immunizations and medications and perform other procedures as needed, including but not limited to: EKGs, suture removals, wound dressing changes, PPD application and interpretation, and blood pressure checks.
- Perform nurse visits independently and within the registered nurse scope of practice as outlined by the Massachusetts Board of Nursing.
- Delegates activities to a non-nurse (i.e., medical assistant) within the parameters outlined by the Massachusetts Board of Nursing https://www.mass.gov/info-details/five-rights-of-delegation.
- Adheres to productivity goals such as those around panel size, patient engagement, and nurse visits.
- Maintain accurate and timely documentation of all nursing care and patient communication, including phone notes, nursing visit notes and immunization or medication administration documentation, in accordance with NSCH policies and protocols.
- Complete documentation within the care management platform(s) as directed by manager.
- Ensures patient quality and coding gaps are addressed and collaborates with team to help meet population health/quality goals.
- Provide coverage at any of the NSCH clinical sites as needed.
- Performs additional duties to fulfill NSCH's mission, as assigned.
Requirements
- College degree from an accredited school of nursing with current license to practice as an RN in the Commonwealth of Massachusetts.
- Current CPR certification.
- Experience working as an RN in a community health center or related setting strongly preferred.
- Ability to function effectively both as a leader, and a member of a team as well as a proven ability to work independently.
- Ability to work in a culturally-diverse environment.

Responsibilities
- Provides outstanding customer service, both to internal and external customers
- Greet patients over the phone/and in person in a friendly and professional manner and providing them with the information and/or services needed
- Answer multiple calls simultaneously providing assistance with making or rescheduling appointments, directing calls to other staff, and taking accurate and detailed messages to ensure that callers receive the information they need
- Verify and update patient demographic and insurance information
- Maintain an organized and clean work areas and waiting room
- Provides translation services as needed
- Contacts patients as needed, via letter, telephone or patient portal
- Schedules patients as needed
- Adhering to HIPAA rules including maintaining strict patient confidentiality.
- Copies and faxes documents as needed
- Performing additional duties to fulfill NSCH's mission, as assigned.
Requirements
- High School diploma or equivalent
- A minimum of two (2) years’ of customer service experience preferably in a medical setting or office environment
- English and Spanish or Portuguese is a plus.
- Demonstrated work history of reliability, good attendance and positive attitude
- Detail-oriented and thorough
- Willingness and ability to learn new technologies; phone system, GE Centricity for medical appointments, Dentrix for dental appointments
- Warm and professional telephone manner; good memory and patience

School Based Behavioral Health Clinician Hybrid In-Person and Telehealth

We currently have an opening for a School Based Behavioral Health Clinician Hybrid In-Person and Telehealth provider. Under the direction of Director of School Based Health, the School Based Behavioral Health Clinician Hybrid In-Person and Telehealth provider is a Masters level licensed clinician provides both tele-behavioral health and in-person behavioral health services to children, adolescents and families in Peabody and Salem primary, middle and high schools. This position is a full-time school year position, with additional 80 summer hours for continuation of services.

Duties/Responsibilities:
- Provide crisis intervention, intake, diagnostic assessment, treatment planning to patients and outpatient psychotherapy services (individual, families, group).
- Provide consultation to primary care and school staff pertaining to the behavioral health needs and treatment goals of their patients.
- Network/collaborate with community leaders, health care providers, community-based organizations, and others who contribute to the ongoing assessment of the health needs of this group of patients.
- Conduct community outreach to: provide information/make referrals to/coordinate services; identify community resources that provide needed support for patients.
- Shares best practices and expertise with both school and NSCH staff.
- Works in collaboration and under the direction of the school-based site manager (if applicable).
- Meet documentation standards in a timely fashion using the electronic health record for each patient contact.
- Attends in-service training for ongoing licensure requirement and complies with all policies for compliance and documentation.
- Participate in supervision, team and staff meetings, on committees and task forces, as required.

Requirements

Responsibilities
- Works as an integral member of the PCT, which includes careful attention to customer service, population management, appropriate delegation and participation in the quality initiatives of the health center. Because of the inherent authority and responsibility of the position, the physician is expected to display exemplary communication skills and to lead by example.
- Conducts a program of care within their discipline’s scope of practice and in collaboration with physician staff, with responsibility for examination, diagnosis and treatment of patients. Obtains history and physical examinations of the patients in an ambulatory setting. Utilizes laboratory and other diagnostic services as appropriate.
- Renders services to patients of NSCH in a competent, professional and ethical manner; makes a diagnosis and administers treatments based on evidence-based medicine whenever possible but always within accepted standards of care.
- The advanced practitioner will prescribe buprenorphine containing medications as needed for the safe care of patients living with opioid use disorder.
- Seeks outside advice and consultation as necessary; refers for specialty care and for confirmation of diagnosis and treatment when appropriate.
- Participates in health promotion and disease prevention activities including screening, immunization and health education events sponsored by NSCH.
- Takes part in quality assurance and patient care assessment activities. PCMH relies on each PCT to act on data provided to it, to continuously improve the quality of care.
- Scope of practice for invasive procedures will be established on the date of hire. New procedures can only be added with the written approval of the (associate) medical director and the CCO.
- Adheres to the documentation standards set for NSCH staff.
- Performs emergency treatment consistent with established policies and procedures accepted standards of care.
- Assumes "on call" responsibilities for NSCH.
- Participates in extended access to patient care as needed.
- Participates in and complies with all policies, procedures and reimbursement plans related to MassHealth and other state and federal reimbursement programs or commercial payors with which NSCH may contract or affiliate.

Responsibilities:
- Provide Level 1 support for Azure, Active Directory, Meraki, VPN, and EMR related issues to end-users.

Requirements
- Associate's degree in Computer Science or a related field, or equivalent work experience.
- 1-2 years of experience in IT support with a focus on Azure, Active Directory, Meraki, VPN, EMR systems like Epic, and HIPAA regulations.
- Strong knowledge of Azure, Active Directory, Meraki, VPN, and EMR systems concepts, including account management, group policy, security, and authentication protocols. Epic EMR is a plus.
- Familiarity with HIPAA regulations and ability to maintain compliance in all IT activities.
- Strong problem-solving skills and ability to troubleshoot complex technical issues.
- Azure, Active Directory, Meraki, VPN, and EMR certification is a plus.

Duties/Responsibilities:
- Provide crisis intervention, intake, diagnostic assessment, treatment planning to patients and outpatient psychotherapy services (individual, families, group).
- Provide consultation to primary care and school staff pertaining to the behavioral health needs and treatment goals of their patients.
- Network/collaborate with community leaders, health care providers, community-based organizations, and others who contribute to the ongoing assessment of the health needs of this group of patients.
- Conduct community outreach to: provide information/make referrals to/coordinate services; identify community resources that provide needed support for patients.
- Shares best practices and expertise with both school and NSCH staff.
- Works in collaboration and under the direction of the school-based site manager (if applicable).
- Meet documentation standards in a timely fashion using the electronic health record for each patient contact.
- Attends in-service training for ongoing licensure requirement, and complies with all policies for compliance and documentation.
- Participate in supervision, team and staff meetings, committees and task forces, as required.

Requirements

Responsibilities
- Works as an integral member of the PCT, which includes careful attention to customer service, population management, appropriate delegation and participation in the quality initiatives of the health center. Because of the inherent authority and responsibility of the position, the physician is expected to display exemplary communication skills and to lead by example.
- Conducts a program of care within their discipline’s scope of practice and in collaboration with physician staff, with responsibility for examination, diagnosis and treatment of patients. Obtains history and physical examinations of the patients in an ambulatory setting. Utilizes laboratory and other diagnostic services as appropriate.
- Renders services to patients of NSCH in a competent, professional and ethical manner; makes a diagnosis and administers treatments based on evidence-based medicine whenever possible but always within accepted standards of care.
- The advanced practitioner will prescribe buprenorphine containing medications as needed for the safe care of patients living with opioid use disorder.
- Seeks outside advice and consultation as necessary; refers for specialty care and for confirmation of diagnosis and treatment when appropriate.
- Participates in health promotion and disease prevention activities including screening, immunization and health education events sponsored by NSCH.
- Takes part in quality assurance and patient care assessment activities. PCMH relies on each PCT to act on data provided to it, to continuously improve the quality of care.
- Scope of practice for invasive procedures will be established on the date of hire. New procedures can only be added with the written approval of the (associate) medical director and the CCO.
- Adheres to the documentation standards set for NSCH staff.
- Performs emergency treatment consistent with established policies and procedures accepted standards of care.
- Assumes "on call" responsibilities for NSCH.
- Participates in extended access to patient care as needed.
- Participates in and complies with all policies, procedures and reimbursement plans related to MassHealth and other state and federal reimbursement programs or commercial payors with which NSCH may contract or affiliate.
- Participation in and compliance with all policies and procedures related to Medicare, Medicaid, other federal and state reimbursement programs, Blue Cross/Blue Shield, and the payment plan of any commercial insurer, health maintenance organization, preferred provider organization, accountable health plan, or other health benefit program with which NSCH may contract or affiliate.

- Current NP/PA Massachusetts License.
- Certification in respective specialty.

Responsibilities:
- Provide direct patient care within the scope of the RN license.
- In addition to chronic pain management, the chronic pain RN Care Manager will be expected to coordinate screening to track some chronic disease measures while working closely with the primary care team.
- Supports the primary care team as staffing needs dictate including, but not limited to triage, nursing desktop management, and chronic disease management within the scope of the RN license
- Assist in the development of clinical policies and patient flow design, with particular attention to protocols around screening and risk stratification.
- Participate in developing the workflow for communication with PCPs as well as with Suboxone certified physicians.
- Maintain and deepen our relationship with our most common behavioral health partners
- Document patient encounters within 48 hours in the NSCH EHR (Epic).
- Ensure documentation is clear, objective and reflects the delivery of safe, quality care.
- Utilize DRVS registries and EHR data to optimize the medical care of patients in treatment for substance use disorder, enhancing preventive care and the ongoing treatment of chronic issues.
- Establish and maintain an ongoing positive working relationship with all members of the care team.
- Collaborate with other staff regarding pertinent patient issues including primary care concerns, chronic disease management, and chronic pain concerns.
- Respect and maintain patient confidentiality in all aspects for care including electronic information.
- Respect patients taking daily medication for pain and/or in substance use treatment and exhibit a supportive, non-judgmental attitude.
- Contribute to the PCMH based efforts to care for the patient.

Requirements
- RN with current MA license
- 3 – 5 years of experience with preference given to those working in community health or with chronic pain.

- Case management experience strongly preferred with experience in substance use disorder treatment services or chronic pain management, although initial training and ongoing education will be provided.

Responsibilities:
- Assesses an individual’s basic health status and related patient care needs for patients who call the clinic or who present for scheduled or unscheduled care
- Analyzes and interprets data and making informed judgments related to patient care
- Evaluates outcomes of basic nursing interventions and initiates changes in care plans
- Collaborates and communicates with primary care providers and other care team members to ensure quality and continuity of care, including participation of huddles to help ensure a smooth workday and maximize time with patients in the clinic

Responsibilities:
- Communicate with members of patient’s care team outside of the health center as needed, which includes specialists or visiting nurses, to help with coordination of care.
- Serve as a liaison between NSCH and other care management programs and community-based organizations.
- In collaboration with CHWs, ensure patient access to local community resources and help link care teams/patients with the appropriate resources.
- Connect primary care team patients with needed behavioral health services, flexible services, Community Partners, respite care and other services.
- Guide care team in accessing durable medical equipment, health services, and other community-based social services for patients.
- Provide care coordination, facilitate care transitions, support the completion of referrals, and/or provide and confirm appropriate follow-up with care providers both within NSCH and with collaborating community care providers.
- Oversee hospital and emergency department discharge workflows for patients on assigned panel.
- Engage patients at their homes or other locations where the member may be located.
- Meet patients while they are in the hospital, when appropriate, to provide education and support around the discharge process and transition members into care management.
- Collaborate with specialty nursing care teams to connect patients with Hep C/HIV care, substance use disorder treatment, chronic disease/diabetes care, and anticoagulation management.
- Perform job duties of the PC Team RN as needed, which include:
- Collaborate and communicate with team primary care providers and other care team members to ensure quality and continuity of patient care on the primary care team
- Participate and coordinate team huddles
- Manage clinically related phone calls for the team including but not limited to: triage of same day appointments, abnormal labs, medication questions, and hospital discharges
- Review refill requests from pharmacy and patients based on medication refill protocol; Reconcile patient medication lists and attach pertinent information necessary for provider to authorize the refill
- Complete patient forms to the furthest extent possible, including prior authorization forms, request for VNA services, etc.
- Assess and evaluate patient care needs
- Analyze and interpret data and make informed judgments related to patient care.
- Evaluate outcomes of basic nursing interventions and initiate changes in care plans.
- Perform nursing interventions
- Administer immunizations and medications and perform other procedures as needed, including but not limited to: EKGs, suture removals, wound dressing changes, PPD application and interpretation, and blood pressure checks.
- Perform nurse visits independently and within the registered nurse scope of practice as outlined by the Massachusetts Board of Nursing.
- Delegates activities to a non-nurse (i.e., medical assistant) within the parameters outlined by the Massachusetts Board of Nursing https://www.mass.gov/info-details/five-rights-of-delegation.
- Adheres to productivity goals such as those around panel size, patient engagement, and nurse visits.
- Maintain accurate and timely documentation of all nursing care and patient communication, including phone notes, nursing visit notes and immunization or medication administration documentation, in accordance with NSCH policies and protocols.
- Complete documentation within the care management platform(s) as directed by manager.
- Ensures patient quality and coding gaps are addressed and collaborates with team to help meet population health/quality goals.
- Provide coverage at any of the NSCH clinical sites as needed.
- Performs additional duties to fulfill NSCH's mission, as assigned.
Requirements

Responsibilities
- NSCH hires board-certified physicians because of their advanced medical education and clinical training. To that end, physicians at NSCH have the highest level of medical expertise on our care teams. Physicians are expected to support advanced practitioner (AP) staff, nursing staff, and other clinical support staff by providing the safest and highest quality of patient care possible. Physicians will support all other clinical staff with clinical assessments, diagnostic clarification, and treatment plan development on an as-needed basis.
- Works as an integral member of the PCT which includes careful attention to customer service, population management, appropriate delegation and participation in the quality initiatives of the health center. Because of the inherent authority and responsibility of the position, the physician is expected to display exemplary communication skills and to lead by example.
- Conducts a program of care with responsibility for examination, diagnosis and treatment of patients. Obtains history and physical examinations of the patients in an ambulatory setting. Utilizes laboratory and other diagnostic services as appropriate.
- The physician will prescribe buprenorphine containing medications as needed for the safe care of patients living with opioid use disorder.
- Renders services to patients of NSCH in a competent, professional and ethical manner; makes a diagnosis and administers treatments based on evidence-based medicine whenever possible but always within accepted standards of care.
- Seeks outside advice and consultation as necessary; refers for specialty care and for confirmation of diagnosis and treatment when appropriate.
- Participates in health promotion and disease prevention activities including screening, immunization and health education events sponsored by NSCH.
- Takes part in quality assurance and patient care assessment activities. PCMH relies on each PCT to act on data provided to it, to continuously improve the quality of care.
- Physicians with osteopathic training perform manipulation within the scope of their practice.
- Scope of practice for invasive procedures will be established on the date of hire. New procedures can only be added with the written approval of the (associate) medical director and the CCO.
- Adheres to the documentation standards set for NSCH staff.
- Performs emergency treatment consistent with established policies and procedures accepted standards of care.
- Assumes "on call" responsibilities, evening and weekend coverage for NSCH.
- Participates in extended access to patient care as needed.

Responsibilities:
- Acts as the primary support contact to application’s end-users for HIM activities such as potential duplicate patients, merges, chart corrections, overlays, data integrity, interface errors, system maintenance, and other HIM activities.
- Perform in-depth analysis and data collection of report details and other technical issues associated with Epic HIM.
- Perform maintenance tasks (error queues, reviews, etc.) and participates in development, execution, and sign off of system testing.
- Develop and maintain detailed documentation on system configurations and technical components.
- Acts as system admin to work with managers to submit requests for Epic accounts, access, and training tracts for end users.
- Works with and troubleshoots for end users’ Epic connectivity issues such as logging into Citrix and signing into Epic.
- Identify potential system enhancement needs, and introduce best practice options for future state workflows and processes.
- Identify issues that arise in their application area as well as issues that impact other application teams and working to resolve them.
- Guide and create workflow, including design, build, and testing the system.
- Analyze technical issues associated with EMR.
- Serves as a liaison between end users’ workflow needs and implementation staff and assists the end users in connecting with higher level Epic support.
- Maintain regular communication with EMR representatives, as well as participating in weekly project team meetings.

- EMR Certification for Epic HIM is a plus.

RESPONSIBILITIES
- Provides intake, diagnostic assessment, treatment planning to primary care North Shore Community Health patients with chronic medical, behavioral health and/or substance use issues
- Provides outpatient psychotherapy services (individual, families, group).
- Provide crisis intervention
- Provide consultation to primary care and dental staff pertaining to the behavioral health needs of their patients.
- Shares best practices and expertise to NSCH staff
- Meet documentation standards in a timely fashion using the electronic health record for each patient contact.
- Attends in-service training for ongoing licensure requirement
- Complies with all policies for compliance and documentation.
- Participate in supervision and team and staff meetings, as appropriate.
- Participates on committees and task forces as appropriate.

Requirements
- LICSW or PsyD required
- Demonstrated strong communication skills with individuals and/or groups

Responsibilities:
Patient Care and Work Flows
- Fulfills all responsibilities of the physician or advanced practitioner role described in the applicable physician/advanced practitioner job description.
- Coordinates and oversees the daily flow of work, and helps to manage providers’ work schedules, assignments, and coverage.
- Collaboratively develops, implements and revises policies, procedures and protocols affecting medical practice, quality of care, and patient safety in order to meet standards of care, improve work efficiency, and improve the patient experience.
- Establishes coordinated communication between all care team roles to ensure timely and appropriate response to patient needs.
Personnel Management
- Directs and supports employees’ daily work activities. Participates in the following employment actions in collaboration with Human Resources: interviewing, hiring, on-boarding, staffing, training, and corrective action. These duties may be in collaboration with the site triad of leadership (nurse manager and practice manager)
- Conducts performance reviews on all direct reports and contributes to the performance reviews of interdisciplinary team members.
o Provides timely feedback to staff on their job performance
o Ensures staff’s adherence to policy, procedures and workflows of NSCH
o Trains management staff to ensure they are leading their respective teams in accordance with performance
expectations, goals set for their jobs, NSCH values, policies, procedures and workflows.
- In collaboration with the Director of Compliance, responds to, investigates, and resolves staff and patient questions and grievances
- Ensures provider compliance with clinical policies and procedures including the proper usage of EMR, fulfillment of documentation expectations, and timely medical billing practices
- Monitors quality of provider care utilizing chart reviews and provides clinical consultation and judgment where appropriate.
- Coordinates and executes peer reviews in accordance with NSCH policy
- Responsible for ensuring compliance to regulatory requirements including HRSA, HPAA, DPH, and FTCA.
Site and Program Management
- Collaborates with Operations Team and the triad of site leadership to create and monitor providers' schedules ensuring appropriate patient access to services
- Participates in the development of Primary Care Team assignments and patient empanelment.
- In collaboration with the site triad of leadership ensures timely submission of scheduled time off requests for primary care staff in accordance with NSCH policies and procedures.
- Ensures short-term schedule adjustments are made for provider vacations and/or absences and that any rescheduling of patient visits occurs in a timely way.
- Helps to plan and participates in site staff meetings in collaboration with the clinical nurse manager and practice manager
- Leads continuous improvement projects which are in alignment with NSCH’s annual quality improvement and strategic plans
- Collaborates with the CCO, other Clinical Directors and NSCH staff on grant-specific projects, including the development of work plans, monitoring quality measures, and implement measures to improve outcomes related to various clinical measures
Fiscal/Budget Management
- In collaboration with Leadership Team and Finance staff, is responsible for planning, monitoring, and managing department budget.
- In coordination with operational and nursing leadership, manage staffing levels (i.e., personnel cost), non-personnel spending, productivity (i.e., revenue), and quality metrics (i.e., revenue and cost).
- Assist leadership in evaluating existing programs and in developing new ones.
- Adhere to HIPAA rules including maintaining strict patient confidentiality.
- Perform additional duties as requested/required to fulfill NSCH’s mission.
- Attends periodic staff meetings and training sessions.
- Complies with proper paperwork procedures, prompt follow-up communications and departmental policies.
Requirements
- The Primary Care Medical Director is a full-time position
- MD/DO or NP licensed in state of Massachusetts and board certified in respective primary care specialty
- At least 5 years of experience as a physician/NP/PA, preferably in a community health center or clinical settings focused on care of the medically underserved
- If an NP, must have independent practice authority in the state of Massachusetts
- Must have strong supervisory skills and experience; ability to manage and lead a broad range of personnel and gain their respect and trust.

Responsibilities:
Patient Care and Work Flows
- Fulfills all responsibilities of the physician or advanced practitioner role described in the applicable physician/advanced practitioner job description.
- Coordinates and oversees the daily flow of work, and helps to manage providers’ work schedules, assignments, and coverage.
- Collaboratively develops, implements and revises policies, procedures and protocols affecting medical practice, quality of care, and patient safety in order to meet standards of care, improve work efficiency, and improve the patient experience.
- Establishes coordinated communication between all care team roles to ensure timely and appropriate response to patient needs.
Personnel Management
- Directs and supports employees’ daily work activities. Participates in the following employment actions in collaboration with Human Resources: interviewing, hiring, on-boarding, staffing, training, and corrective action. These duties may be in collaboration with the site triad of leadership (nurse manager and practice manager)
- Conducts performance reviews on all direct reports and contributes to the performance reviews of interdisciplinary team members.
o Provides timely feedback to staff on their job performance
o Ensures staff’s adherence to policy, procedures and workflows of NSCH
o Trains management staff to ensure they are leading their respective teams in accordance with performance
expectations, goals set for their jobs, NSCH values, policies, procedures and workflows.
- In collaboration with the Director of Compliance, responds to, investigates, and resolves staff and patient questions and grievances
- Ensures provider compliance with clinical policies and procedures including the proper usage of EMR, fulfillment of documentation expectations, and timely medical billing practices
- Monitors quality of provider care utilizing chart reviews and provides clinical consultation and judgment where appropriate.
- Coordinates and executes peer reviews in accordance with NSCH policy
- Responsible for ensuring compliance to regulatory requirements including HRSA, HPAA, DPH, and FTCA.
Site and Program Management
- Collaborates with Operations Team and the triad of site leadership to create and monitor providers' schedules ensuring appropriate patient access to services
- Participates in the development of Primary Care Team assignments and patient empanelment.
- In collaboration with the site triad of leadership ensures timely submission of scheduled time off requests for primary care staff in accordance with NSCH policies and procedures.
- Ensures short-term schedule adjustments are made for provider vacations and/or absences and that any rescheduling of patient visits occurs in a timely way.
- Helps to plan and participates in site staff meetings in collaboration with the clinical nurse manager and practice manager
- Leads continuous improvement projects which are in alignment with NSCH’s annual quality improvement and strategic plans
- Collaborates with the CCO, other Clinical Directors and NSCH staff on grant-specific projects, including the development of work plans, monitoring quality measures, and implement measures to improve outcomes related to various clinical measures
Fiscal/Budget Management
- In collaboration with Leadership Team and Finance staff, is responsible for planning, monitoring, and managing department budget.
- In coordination with operational and nursing leadership, manage staffing levels (i.e., personnel cost), non-personnel spending, productivity (i.e., revenue), and quality metrics (i.e., revenue and cost).
- Assist leadership in evaluating existing programs and in developing new ones.
- Adhere to HIPAA rules including maintaining strict patient confidentiality.
- Perform additional duties as requested/required to fulfill NSCH’s mission.
- Attends periodic staff meetings and training sessions.
- Complies with proper paperwork procedures, prompt follow-up communications and departmental policies.
Requirements
- The Primary Care Medical Director is a full-time position
- MD/DO or NP licensed in state of Massachusetts and board certified in respective primary care specialty
- At least 5 years of experience as a physician/NP/PA, preferably in a community health center or clinical settings focused on care of the medically underserved
- If an NP, must have independent practice authority in the state of Massachusetts
- Must have strong supervisory skills and experience; ability to manage and lead a broad range of personnel and gain their respect and trust.
- Must have the ability to respond quickly and purposefully to routine and urgent situations.
- Ability to work independently
- Initiative-taking with strong organizational and time management skills
- Must be comfortable operating in a collaborative, shared leadership environment.
- Demonstrates interest and enjoyment of work. Ability to inspire others.
- Demonstrates an exceptionable ability to listen without bias, think critically to discern the key salient points and focus on the tasks that will move the goals of the team forward.
- Preference will be given to qualified bilingual/bicultural individuals who can speak Spanish and/or Portuguese
- Ability to travel to other NSCH sites for meetings and clinical coverage as needed is required.
- Evening, weekend, and after hours on-call availability may be required.
