Community Health Worker – Care Management
Opportunity Overview
North Shore Community Health (NSCH) is an industry leader in exceptional delivery of community healthcare. We are looking for a Community Health Worker for Care Management (CHW – CM). The Community Health Worker for Care Management (CHW – CM) will provide culturally appropriate health education, counseling, case management, follow up, and advocacy for NSCH patients with complex health needs. The CHW- CM collaborates with the Primary Care Team, ACO team and other staff to help coordinate services and improve access to care with a focus on patients who are identified for care management through ACO programs. The CHW-CM also performs community outreach in order to facilitate linkages with community agencies to address barriers related to social determinants of health.

Duties/Responsibilities:
- Provides culturally competent support services for patients depending on individual needs and preferences. These include but are not limited to:
- Provides education/information/assistance in obtaining access to primary care, behavioral health care and other human services
- Management of urgent non-medical issues (connecting urgently to social service agencies for example around housing or food)
- Conducts outreach calls to encourage patients and caregivers to participate in specific health care programs
- Conducts and documents relevant patient assessments as designated by health center and ACO programs.
- Develops and implements outreach plans in collaboration with team colleagues, based on individual, family, and community needs, strengths and resources
- Uses a range of outreach methods to engage individuals and groups in diverse settings (for example: phone, email, text, face-to-face, etc.)
- Identifies barriers to care and helps identify solutions, problem-solving within the health center and with other agencies, as needed
- Provides patient support and advocacy
- Initiates and sustains trusting relationships with individuals, families, social networks and primary care team
- Provides informal supportive counseling and encouragement
- Provides direct assistance with completion of forms and making appointments
- Communicates with patients’ primary care and/or behavioral health providers
- Coordinates and/or provides interpreting for patient appointments within and outside of NSCH on an as-needed basis
- Collaborates with community agencies, for example, behavioral health community partners and long-term services and supports, to improve care coordination
- Provides ongoing monitoring, follow-up and discharge planning, as appropriate, under the direction of the Nurse Manager, ACO team members, or other care team members.
- Provides education on chronic disease management, comprehensive preventive care, and self-management goals as directed/provided by the PCP, RN, BH provider or other appropriate team member.
- Ensures access to or develops/maintains updated community support services and resource lists for patients and staff
- Conducts community outreach to:
- Provide information/make referrals to/coordinate services
- Identify community resources that provide needed support for patients
- Provide health education to individuals and groups on prevention and public health risks and problems
- Educate staff/providers about target population and community characteristics and needs
- Participates in team and staff meetings for NSCH and for the ACO
- Works with individuals, family, community members, primary CM and primary care team to address issues that may limit opportunities for healthy behavior.
- Meets documentation standards in a timely fashion using the electronic health record and/or the care management platform, under the direction of the Nurse Manager or other designated manager.
- Maintains record of patient contacts, as required
- Serves as a resource for other staff members around ACO care management workflows
- Serves as an expert on community resources for patients and helps troubleshoot relevant patient cases along with other team members.
- Keeps current with ACO communication/updates around CM and helps foster successful communication between NSCH and the ACO.
- Uses appropriate technology, such as computers, for work-based communication according to North Shore Community Health requirements.
Requirements
- Four or more years’ experience and training in a community health center setting, preferred; and/or related experience in health/human services, social services agency
- BA in health or human services preferred, not required
- Demonstrated strong communication skills with individuals and/or groups
- Fluency in Spanish and/or Portuguese preferred, not required
- Experience working with patients with chronic and behavioral health needs
- Strong computer skills and ability to quickly adapt to using an electronic health record and Microsoft Office applications
- Maintains strict adherence to the NSCH Confidentiality policy

We have an opening for a Physician. Under the general direction of the Chief Medical Officer (CMO) but the direct supervision of the Medical Director, the physician will provide continuing and comprehensive health care for the individual and family across all ages and genders. Within the context of Patient Centered Medical Home (PCMH) policies and procedures and within standards of care for each respective specialty (Family Medicine, Internal Medicine, Pediatrics, Psychiatry and Gyn), the clinician will exercise individual initiative and independent action to ensure high quality and comprehensive primary care. The two central tenets of PCMH lie in team based care and care coordination. The physician is expected to function as integral part of the Primary Care Team, sometimes as a leader and sometimes as engaged member. As a team member at our health center, you will work with staff who are racially, ethnically and culturally diverse, so being culturally competent is required. A sense of humor is also very important!
Responsibilities
• Works as an integral member of the PCT, which includes careful attention to customer service, population management, appropriate delegation and participation in the quality initiative of the health center. Because of the inherent authority and responsibility of the position, the physician is expected to display exemplary communication skills and to lead by example.
• Conducts a program of care with responsibility for examination, diagnosis and treatment of patients. Obtains history and physical examinations of the patients in an ambulatory setting. Utilizes laboratory and other diagnostic services as appropriate.
• Renders services to patients of NSCH in a competent, professional and ethical manner; makes a diagnosis and administers treatments based on evidence-based medicine whenever possible but always within accepted standards of care.
• Seeks outside advice and consultation as necessary; refers for specialty care and for confirmation of diagnosis and treatment when appropriate.
• Maintains appropriate and timely medical records on patients under care.
• Performs emergency treatment consistent with established policies and procedures.
• Assumes "on call" responsibilities for NSCH.
• Participates in extended access to patient care as needed.
• Attends scheduled meetings in hospitals and NSCH. Each Physician is expected to maintain good standing within his or her specialty department either at NSMC or at Beverly Hospital. Physicians working in Salem and Peabody are expected to attend 6 meetings per year with NSHS, the PHO to which NSCH belongs.
• Attends scheduled meetings, maintains good standing, and complies with all policies and procedures of NSCH and any hospital or facility where privileges have been granted.
• Participates in and complies with all policies, procedures and reimbursement plans related to MassHealth and other state and federal reimbursement programs or commercial payors with which NSCH may contract or affiliate,
• Participates in health promotion and disease prevention activities including screening, immunization and health education events sponsored by NSCH.
• Takes part in quality assurance and patient care assessment activities. PCMH relies on each PCT to act on data provided to it, to continuously improve the quality of care.
• Physicians with osteopathic training perform manipulation within the scope of their practice.
• Scope of practice for invasive procedures will be established on the date of hire and reviewed annually on or near the anniversary of the date of hire. New procedures can only be added with the written approval of the (associate) medical director and the CMO.
• Participation in and compliance with all policies and procedures related to Medicare, Medicaid, other federal and state reimbursement programs, Blue Cross/Blue Shield, and the payment plan of any commercial insurer, health maintenance organization, preferred provider organization, accountable health plan, or other health benefit program with which NSCH may contract or affiliate.
• Must comply with all NSCH policies and procedures; all bylaws, rules, regulations and policies of any hospital or facility of which you are granted privileges; and all applicable state and federal law
• Fulfills the expectations of the physician position including other duties as needed.

Requirements
• MD required with current Massachusetts License

Duties/Responsibilities:
- Provides outstanding customer service, both to internal and external customers
- Screens patients according to established protocol including but not limited to height, weight, urine testing, hematocrits, vision testing, blood pressure, PHQ-9
- Prepares paperwork for referrals and provides translation services as needed
- Obtains tests as delineated, and sets up for procedures as listed under the MA competency checklist
- Performs the tasks within the EMR as listed in the MA competency checklist
- Participates in the orders, hospital discharge and ER tracking
- Clean and stock exam rooms, maintain sterilization statistics, and an inventory control system
- Perform quality assurance tests as necessary
- Answers phone or EMR message when needed. Contacts patients, via letter, telephone or patient portal and schedule patients as needed
- HIPAA: Maintains integrity and completeness of medical record and patient confidentiality
- Copies and faxes documents as needed
- Manages durable medical equipment requests. Assists with visiting nurse referrals and prior authorizations
- Performing additional duties to fulfill NSCH's mission, as assigned
Requirements
- Graduate of a Medical Assistant program preferred
- Experience working in a medical field as a CNA, EMT, Phlebotomy and other related fields
- Students in a Medical field
- Completion or willingness to complete Basic Life Support training (CPR)
- Ability to work effectively as part of the team
- Willingness to be flexible about work assignments
- Strong interpersonal and communication skills
- The ability to float between the three sites and work from home at times
- Bilingual especially Spanish or Portuguese a plus

Provider Scheduling Assistant/ Float PSR

Under the direct supervision of the Sr Manager of Facilities and Patient Experience, the Provider Scheduling Assistant/Float PSR is responsible for scheduling primary care, behavioral health, M.A.T., C.H.W., Outreach and enrollment, Nursing, and SBHC resources at the Salem Family Health Center, Peabody Family Health Center, and Gloucester Family Health Center. The Assistant /Float PSR is responsible for maintaining optimal provider and medical assistant staffing levels and works closely with Site Medical and Behavioral Directors to ensure appropriate patient access is maintained. This position will act as a float PSR and as such will accurately perform activities related to the smooth functioning of the Front Desk functions. As a team member at our health center, you will work with staff who are racially, ethnically and culturally diverse, so being culturally competent is required. - Create providers schedules in collaboration with Site Medical Directors in accordance with NSCH’s Staffing Guidelines.
- Creates and Maintains accurate scheduling & telephonic templates in EMR for primary care, behavioral health, M.A.T., C.H.W., Outreach and Enrollment, Nursing, and SBHC.
- Ensures that clinic schedules are open 5 weeks in advance in EMR.
- Maintains Saturday Schedule for Salem Walk-in hours from 9-1 to ensure that one provider, one nurse, one medical assistant and two PSR’s are scheduled: sends email reminders to Saturday staff scheduled to work.
- Updates and maintain AMION.COM in real-time to reflect all onsite staff including providers, nurses, MA’s and PSR’s working in each of the sites for any given day. Additionally, notate who is away, sick and / or on call.
- Follows standardized process for opening provider templates across sites both for onsite and telephonic
- Blocks Monthly Provider meeting and Site meeting for all three sites for scheduling purposes
- Maintains Monthly Calendar for Float Staff, Chat NPs and creates Lab Template for Salem, Peabody, & Gloucester
- Creates Flu/Covid clinic and pediatric clinic templates for Salem, Peabody, and Gloucester.
Patient Services Representative Responsibilities
- Provides outstanding customer service both to internal and external customers.
- Greet patients over the phone/and in person in a friendly and professional manner and providing them with the information and/or services needed.
- Verify and update patient demographic and insurance information.
- Maintain an organized and clean work areas and waiting room.

RESPONSIBILITIES
- Provides direct supervision and effective leadership through proper development and training of clinical nurses and medical assistants

Coordinates and participates in training initiatives including orientation, annual competency assessment and continuing education programs. As well as implement training initiatives through coaching of staff and performance monitoring
- Working with DON plan, implement and evaluate quality improvement activities
- Reviews medical assistants and nurses’ documentation regularly to ensure proper provision of/documentation of care
- Design, evaluate and monitor practice workflows including providing guidance to staff in meeting productivity goals
- Oversees supply and inventory of vaccines, medications and nursing supplies, including emergency medications/equipment
- Provides clinical coverage at any of the clinical sites as needed
- In collaboration with the Director of Nursing hiring, orienting, evaluating and disciplining medical assistants and nursing staffing as needed
- Manages personnel budget including payroll variances

Requirements
- RN with current Massachusetts license, BSN -preferred
- 1-3 years of clinical and programmatic experience needed
- Management and supervisory experience preferred

Duties/Responsibilities
- Conduct research to identify funders and grant opportunities to fund projects and programs as needed that fit the strategic goals of NSCH
- Responsible for the grant writing, editing, team identification, collaborator identification and submission process, as well as follow up process until award, and occasional input on progress and final report narratives.

Duties/Responsibilities:
- Provide administrative support for the EVP/COO and VP of Integration, Wellness, and Outreach. Key duties include:
o Managing the Outlook calendars, including coordination and scheduling of meetings;
o Provide support for meeting preparation such as collecting input for agendas and meeting materials, assisting with preparation of meeting materials (i.e. PowerPoint presentations; agenda documents; handouts, etc.); disseminating meeting materials, booking conference room space, and ensuring appropriate physical and AV set-up;

Duties/Responsibilities:
- Prepares treatment room for patient by following prescribed procedures and protocols.
- Prepares patient for dental treatment by welcoming, comforting, seating, and draping patient.
- Provides information to patients and employees by answering questions and requests.
- Provides instrumentation by sterilizing and delivering instruments to treatment area; positioning instruments for dentist's access; suctioning; passing instruments.
- Provides materials by selecting, mixing, and placing materials on instruments.
- Provides diagnostic information by exposing radiographs; pouring, trimming study casts.
- Helps dentist manage dental and medical emergencies by maintaining CPR certification, emergency drug and oxygen supply, and emergency telephone directory.
- Educates patients by giving oral hygiene, plaque control, and postoperative instructions.
- Documents dental care services by charting in patient records.
- Complies with all infection control procedures and OSHA regulations to maintain a safe and clean working environment, by decontaminating treatment rooms and sterilizing all instruments after each patient.
- In collaboration with the Dental team, maintains operation of dental equipment by completing preventive maintenance requirements; following manufacturer's instructions; troubleshooting malfunctions; calling for repairs; maintaining equipment inventories; evaluating new equipment and techniques.
- In collaboration with the Dental Practice Manager, maintains dental supplies inventory by checking stock to determine inventory level and anticipating needed supplies.
- In collaboration with the Dental Practice Manager, monitors incoming and outgoing lab cases.
- Schedule accurate dental appointments.
- Provides exemplary customer service; treats patients and coworkers with courtesy and discretion. Greets and directs patients and visitors.
- Registers patients in Dentrix and Centricity, verifying that patient's record is up to date and accurate and makes appropriate insurance & patient personal data (address & phone number) changes.
- Relays patient’s complaints (billing, clinical or otherwise) to Lead Dental Assistant.
- Collects payment from patients, applies payments and adjustments to patient accounts in the computer system and provide receipts.

Requirements

Primary Purpose of Position
We are looking for a self-motivated and experienced Director of Practice Management to oversee the daily operations of high-quality, patient-focused patient services across multiple clinical sites to ensure safe, effective and efficient care delivery. The Director of Practice Management will directly oversee clinical site and operations managers such as for our Salem, Peabody, and Gloucester sites as well as our Telehealth and walk-in services. Reporting directly to the EVP/COO, the Director of Practice Management will think proactively and strategically to identify opportunities for continuous improvement and to support the implementation of the organization’s strategic priorities. The Director will be expected to provide leadership and support of staff in a manner that fosters team work and mutual commitment and enthusiasm towards organizational goals and objectives.
Essential Functions
- Work with EVP/COO to champion operational optimization efforts and strategic initiatives at the clinical sites, helping to inform and support approach to communication, building buy-in, and engaging staff at the sites. Work in concert with EVP/COO to ensure continuous improvement efforts are aligned with and prioritized based on the organization’s strategic goals.
- Work with EVP/COO and other members of the Leadership Team to create and refine operational care delivery supports and structures for newer modalities of care including Telehealth and walk-in “urgent care” services.
- Think strategically and proactively on how to better leverage resources to optimize productivity, efficiency, patient experience, and health outcomes and minimize costs. Foster a team culture that embraces collaboration and continuous improvement.
- Help to inform redesign of operational KPIs and dashboard; responsible for monitoring operations against KPIs. Responsible for preparing reports, managing data collection, and analyzing trends, as assigned, to ensure performance is proactively monitored and continuous improvement opportunities are identified and changes are informed by objective data trends.
- Develop budget for staff, supplies and other expenses for the operational and administrative aspects of the clinical sites. This includes working with the Site Managers for non-salary expense assumptions. Monitors spending and proactively assesses opportunities to safely maximize efficiency and minimize costs.
- Directly supervises all Site Managers and Scheduling staff and ensures effective daily operations at all sites. Supervision includes identifying employee goals that are aligned with organizational goals and ensuring team members have the tools, training and information they need to perform at their highest level, continuously assessing onboarding and personnel management practices to ensure operational success.
- Keep staff informed of all clinical site-related and other changes and news going on throughout the organization. Disseminate information to assure assigned staff are familiar with NSCH policies, procedures, and performance expectations of their jobs, including managing time and attendance. Ensure that direct reports who serve as managers are effectively communicating with staff embedded within specific site locations. Train management staff to ensure they are leading their respective teams in accordance with performance expectations, goals set out for their jobs, NSCH values, policies, and procedures.
- Evaluate appropriate staffing levels to ensure safe and effective operations. Establish a work environment that fosters teamwork, cross-team collaboration and open communication. Ensure constant and consistent communication and alignment across the different clinical site and operations managers to ensure cohesive site operations that not only support consistent site daily operations but also align with organizational priorities. Ensure site managers are proactively collaborating with and actively listening to the other managers (i.e. Medical Directors, Nurse Managers, etc.) within the sites, identifying areas of success and opportunity for improvement and engaging staff to find appropriate solutions that are aligned with NSCH’s mission, policies, and organizational priorities.
- Address staff performance through performance evaluation, establishing performance goals, recognition, coaching, counseling, and disciplinary actions in a timely manner and implements action plans and follow-up as needed.
- Oversee the overall workflow and operations at each clinical site, while ensuring protocols are followed and the sites operate in a clean, safe, healthy and friendly environment and ensuring staff at both medical and dental sites have supplies, functional equipment and a clean/safe physical space to do their job.
- Provides day-to-day facility and administrative management and oversight to the site, monitoring the physical environment and calling in appropriate resources to address problems w/ telephones, lighting, electrical, and other facility systems.
- Key participant in NSCH’s Safety Committee. Regularly reviews safety reports, convenes with the committee, analyzes trends and follows up on identified issues.
- Serves as NSCH’s Emergency Preparedness Coordinator.
- Adhere to HIPAA rules including maintaining strict patient confidentiality.
- Attend all required trainings and participate in meetings as required.
- Perform additional duties to fulfill NSCH’s mission, as assigned.

Requirements
- A Bachelor’s degree in health management/administration, business or another related field is required. Master’s degree is preferred.
- 7+ years of experience managing multi-site healthcare operations, preferably in a federally qualified health center or other primary care healthcare setting.
- A proven, team, and mission-motivated, goal-oriented leader
- Proven performance and experience with process improvement projects. Training on Lean/six sigma improvement frameworks is preferred.

Responsibilities
- Assesses an individual’s basic health status and related patient care needs for patients who call the clinic or who present for scheduled or unscheduled care
- Analyzes and interprets data and making informed judgments related to patient care
- Evaluates outcomes of basic nursing interventions and initiates changes in care plans
- Collaborates and communicates with primary care providers and other care team members to ensure quality and continuity of care, including participation of huddles to help ensure a smooth workday and maximize time with patients in the clinic
- Reviews refill requests from pharmacy and patients based on medication refill protocol; Reconciles patient medication lists and attaches pertinent information necessary for provider to authorize the refill
- Administers immunizations and medications and performs other procedures as needed, including but not limited to: EKGs, suture removals, wound dressing changes, ear irrigations, PPD application and interpretation, and blood pressure checks
- Maintains accurate and timely documentation of all nursing care and patient communication, including phone notes, nursing visit notes and immunization or medication administration documentation
- Completes patient forms to the furthest extent possible when needed within the scope of nursing practice including prior authorization forms
- Provides health education to individuals and families to further patient self-management and self-empowerment and to help patients maintain/sustain an optimal level of health
- Communicates with members of patient’s care team outside of the health center as needed, for instance with specialists or visiting nurses, to help with coordination of care
- Manages vaccination supply and ordering, including proper vaccination storage and handling.

Responsibilities
- Works as an integral member of the integrated health care team which includes careful attention to customer service, population management, appropriate delegation and participation in the quality initiatives of the health center.
- Provides for biopsychosocial clinical evaluations, examination, and diagnosis utilizing DSM V diagnoses.
- Collaborates with other providers involved in client care, including nursing, medical providers, behavioral health clinicians, community health workers, social agencies, hospitals, schools, etc.
- Coordinates patient care and serves as a patient advocate.
- Provides psychiatric and psychopharmacology consultation to members of the care team.
- Provides psychopharmacological evaluation and management of patients
- Provides patient and family education
- Seeks professional continuing education to remain current in treatment practice.
- Maintains appropriate and timely medical records on patients under care. Although the NSCHI policy in 2017 is 72 hours, the expectation is for clinically relevant records to be signed and closed within 24 hours.

Requirements
- Master’s Degree in Nursing with certification as a PMHNP or graduate of a certified Physician’s Assistant program
- Two plus years of experience working as a Psychiatric Nurse Practitioner or Physician’s Assistant preferred
- Current licensure to practice psychiatric nursing in Massachusetts.
- Experience in community health care.
- Must have reliable transportation to travel between sites.

Duties/Responsibilities:
- Provides outstanding customer service, both to internal and external customers
- Screens patients according to established protocol including but not limited to height, weight, urine testing, hematocrits, vision testing, blood pressure, PHQ-9
- Prepares paperwork for referrals and provides translation services as needed
- Obtains tests as delineated, and sets up for procedures as listed under the MA competency checklist
- Performs the tasks within the EMR as listed in the MA competency checklist
- Participates in the orders, hospital discharge and ER tracking
- Clean and stock exam rooms, maintain sterilization statistics, and an inventory control system
- Perform quality assurance tests as necessary
- Answers phone or EMR message when needed. Contacts patients, via letter, telephone or patient portal and schedule patients as needed
- HIPAA: Maintains integrity and completeness of medical record and patient confidentiality
- Copies and faxes documents as needed
- Manages durable medical equipment requests. Assists with visiting nurse referrals and prior authorizations

Requirements
- Graduate of a Medical Assistant program preferred
- Experience working in a medical field as a CNA, EMT, Phlebotomy and other related fields
- Students in a Medical field
- Completion or willingness to complete Basic Life Support training (CPR)

Duties/Responsibilities:
- Provide crisis intervention, intake, diagnostic assessment, treatment planning to patients and outpatient psychotherapy services (individual, families, group).
- Provide consultation to primary care and school staff pertaining to the behavioral health needs and treatment goals of their patients.

Duties/Responsibilities:
- Oversee daily operations of SFHC including daily supervising, coaching, evaluating, and developing/training all non-clinical staff to ensure staff perform at their highest potential.
- Evaluate appropriate staffing levels to ensure safe and effective operations, while establishing a work environment that fosters teamwork and open communication.
- Address staff performance through performance evaluation, establishing performance goals, recognition, coaching, counseling, and disciplinary actions in a timely manner and implements action plans and follow-up as needed.
- Disseminate information to assure assigned staff are familiar with NSCH policies, procedures, and performance expectations of their jobs, including managing time and attendance.
- Oversee the overall workflow at SFHC, while ensuring protocols are followed and the site operates in a clean, safe, healthy and friendly environment. Ensure staff at both medical and dental sites have the supplies, functional equipment, and clean/safe physical space to do their job; reports problems and coordinates requests for repairs with the appropriate NSCH staff and/or outside parties.
- Ensure administrative/non-clinical workflows and standard operating procedures (SOPs) are documented and followed including ensuring accurate paperwork and record keeping, checking insurance eligibility, verifying patient demographic information, meeting patient’s needs for insurance enrollment assistance or eligibility for sliding fee discount, collecting co-pays, etc.
- Actively monitor the effectiveness of the SOPs, including gathering feedback from staff, and proactively bring recommendations to appropriate NSCH staff.
- Manage all SFHC patient/customer service complaints and ensure timely reporting of findings while providing outstanding customer service to internal and external customers. Demonstrate commitment to high quality customer service.
- Identify opportunities to better leverage resources to optimize productivity and minimize costs, while focusing on performance and systems improvement.
- Build and maintains ongoing working relationships with all site staff, including clinical, to facilitate positive staff morale and support site-wide goals and initiatives.
- Identify, monitor and work with appropriate staff to resolve IS issues that impact the site’s operations.
- Work with Sr. Manager of Facilities and Patient Experience to inform non-salary expense assumptions for the Salem Site’s budget.
- Ensure procedures are followed to allow patient, state, and insurance notifications to be process timely in compliance with the law requirements.
- Prepare reports and manage data collection, as assigned.
- Adhere to HIPAA rules including maintaining strict patient confidentiality.
- Attend all required enrollment trainings and participate in meetings as required.
- Perform additional duties to fulfill NSCH's mission, as assigned.
Requirements
- A Bachelor’s degree from an accredited institution preferred.
- Fluency in English required. Bilingual in Spanish or Portuguese highly preferred.
- A minimum of two (2) years of experience in a supervisory or management role.
- Supervisory experience in a PCMH ambulatory health care setting; community health center experience preferred.
- A minimum of two (2) years of experience working in a patient centered medical home (PCMH) ambulatory health care setting preferred, preferably a community health center.

Responsibilities
- Assesses an individual’s basic health status and related patient care needs for patients who call the clinic or who present for scheduled or unscheduled care
- Analyzes and interprets data and making informed judgments related to patient care
- Evaluates outcomes of basic nursing interventions and initiates changes in care plans
- Collaborates and communicates with primary care providers and other care team members to ensure quality and continuity of care, including participation of huddles to help ensure a smooth workday and maximize time with patients in the clinic
- Reviews refill requests from pharmacy and patients based on medication refill protocol; Reconciles patient medication lists and attaches pertinent information necessary for provider to authorize the refill
- Administers immunizations and medications and performs other procedures as needed, including but not limited to: EKGs, suture removals, wound dressing changes, ear irrigations, PPD application and interpretation, and blood pressure checks
- Maintains accurate and timely documentation of all nursing care and patient communication, including phone notes, nursing visit notes and immunization or medication administration documentation
- Completes patient forms to the furthest extent possible when needed within the scope of nursing practice including prior authorization forms
- Provides health education to individuals and families to further patient self-management and self-empowerment and to help patients maintain/sustain an optimal level of health
- Communicates with members of patient’s care team outside of the health center as needed, for instance with specialists or visiting nurses, to help with coordination of care
- Manages vaccination supply and ordering, including proper vaccination storage and handling.

- Provides coverage at other NSCH clinical sites or call center as needed.

- College Degree from an accredited school of nursing, current license to practice as an RN in the Commonwealth of Massachusetts

Duties/Responsibilities:
- Provides outstanding customer service, both to internal and external customers
- Screens patients according to established protocol including but not limited to height, weight, urine testing, hematocrits, vision testing, blood pressure, PHQ-9
- Prepares paperwork for referrals and provides translation services as needed
- Obtains tests as delineated, and sets up for procedures as listed under the MA competency checklist
- Performs the tasks within the EMR as listed in the MA competency checklist
- Participates in the orders, hospital discharge and ER tracking
- Clean and stock exam rooms, maintain sterilization statistics, and an inventory control system
- Perform quality assurance tests as necessary
- Answers phone or EMR message when needed. Contacts patients, via letter, telephone or patient portal and schedule patients as needed
- HIPAA: Maintains integrity and completeness of medical record and patient confidentiality
- Copies and faxes documents as needed
- Manages durable medical equipment requests. Assists with visiting nurse referrals and prior authorizations
- Performing additional duties to fulfill NSCH's mission, as assigned
Requirements
- Graduate of a Medical Assistant program preferred
- Experience working in a medical field as a CNA, EMT, Phlebotomy and other related fields
- Students in a Medical field
- Completion or willingness to complete Basic Life Support training (CPR)
- Ability to work effectively as part of the team
- Willingness to be flexible about work assignments
- Strong interpersonal and communication skills
- The ability to float between the three sites and work from home at times
- Bilingual especially Spanish or Portuguese a plus

Telehealth Navigator Manager - Bi-lingual
Opportunity Overview
The COVID-19 emergency changed the dynamic of in-person care and put an emphasis on remote care. By using telecommunications technology and other electronic data, telehealth can assist healthcare systems, organizations, and providers expand access to and improve the quality of healthcare. Using telehealth to deliver and assist with the delivery of healthcare services can reduce or minimize challenges to complete patient encounters, such as transportation issues related to travel to receive medical care. It can also improve monitoring, timeliness, and communications within the healthcare system.
Under the general supervision of the Director of Operations, the Telehealth Navigator Manager will work closely with NSCH team members, telehealth vendors, and patients to enable patients to have equitable access to telehealth services and clinical treatment at NSCH. The functions of this role are conducted in a combination of on-site, community-based and remote environments.

Duties/Responsibilities
- Identify patients scheduled for telehealth visits, focusing on populations with the greatest need for support (e.g. non-English speaking patients), assess ability and needs for a successful video visit, coordinate access to devices, perform patient education, and ensure ongoing support for video visits as needed.
- Work collaboratively with the telehealth team, primary and behavioral health patient teams, Chronic Care Management team, and community-based services, and other clinical team members to increase access to video telehealth services and/or remote patient monitoring services.
- Communicate clearly and effectively with all populations, particularly those without technical experience, about subjects such as device usage.
- Establish contact, build effective trust-based relationships with identified patients.
- Assess each patient’s telehealth history and needs using assigned scripts and tools.
- Stay connected with patients on a need- and resource-appropriate basis to ensure access to telehealth services.
- Adhere to established case management standards of care and provide excellent customer service at all times to all clients.
- Provide technical assistance to patients on use of devices as needed.
- understand current guidelines for telehealth regarding all jurisdictions (Federal, State, Payer, etc.), and remain aware of updates.

Leadership and management
- Ensures compliance with federal and state laws and regulations, accrediting organization, certifying organizations and the standards of NSCH
- Participates in dental program development, grant writing, grant review and reporting in collaboration with other NSCH departments

- Participates in the preparation of grant applications as needed to benefit the continuous growth and development dental department.

Fiscal Responsibility
- Develops operating budget for dental department in collaboration with Dental Director, finance department and health center leadership
- Forecasts provider patient visits based on NSCH data and CHC benchmark data
- Forecasts operating expenses for dental department including supplies and equipment replacement/maintenance.
- Responsible for dental department vendor management to insure optimal performance of negotiated contracts
- Monitors revenue cycle to insure maximized reimbursement for services while insuring adherence to all state, federal and FQHC regulations

- Prepares annual dental department sliding fee scale for essential and non-essential services
- Monitors accuracy and implementation of good faith estimates on all self-pay services
- Develop, set benchmarks and monitor financial metrics including but not limited to:
- No show rate
- Direct Costs Per visit
- Charges per encounter
- Payer mix (per site)
Policy and Procedure Development
• Develops, implements and ensures compliance with NSCH and dental department policies and procedures
• Formulates and recommends policies related to dental staff, quality assessment, and dental education to NSCH leadership
• Ensures compliance to emergency and safety procedures including organizing mock codes and fire drills
Population Health and Quality
- Monitors dental quality metrics in relation to industry benchmarks including but not limited to:
- Case Completion Rate
- Caries at Recall
- Risk assessment of all Dental Patients
- Sealants 6-9 years and 10-14 years (UDS/HRSA measure)
- Topical Fluoride Treatment
- Self-Management Goal Setting
- Types of Services provided
- Percent of patients presenting as Emergencies
- Develops Continuous Quality Improvement Program based on clinical data and quality metrics
- Collaborates with the Director of Quality on developing and implementing specific dental quality improvement projects
- Works with the Dental Director to ensure periodic chart audits; reviews quality metrics
- Develops patient engagement strategies including regular patient satisfaction surveys
Systems integration
- Creates workflows in Electronic Dental Record that support cross departmental communication and collaboration with medical, behavioral health, dental and school-based departments
- Creates and monitors standard supply management system for all dental sites
- Monitors patient schedule for trends in access and capacity to make recommendations for wait lists, expanded hours, increased staffing
- Collaborates with medical directors to develop and implement integrated medical/behavioral and dental health strategies for NSCH patients

Requirements
- A minimum of five years’ experience in dental practice administration
- Ability to collaborate across different practice areas, organizational levels, etc.
- Ability evaluate data to make data-driven decisions
- Demonstrated experience in continuous improvement practices.
- A minimum of three years of leadership in personnel management, and dental operations
- Community Health Center experience preferred
- Experience with Electronic Dental Records
- Demonstrated ability to work in a culturally-diverse environment
- Demonstrated excellent communication, interpersonal, and customer service skills
- Ability to travel to other NSCH sites for meetings and as needed is required.
- Some evening and weekend hours may be required.

Opportunity Overview

We currently have an opening for a School Based Behavioral Health Clinician Hybrid In-Person and Telehealth provider. Under the direction of Director of School Based Health, the School Based Behavioral Health Clinician Hybrid In-Person and Telehealth provider is a Masters level licensed clinician provides both tele-behavioral health and in-person behavioral health services to children, adolescents and families in Peabody and Salem primary, middle and high schools. This position is a full-time school year position, with additional 80 summer hours for continuation of services.

Duties/Responsibilities:
- Provide crisis intervention, intake, diagnostic assessment, treatment planning to patients and outpatient psychotherapy services (individual, families, group).
- Provide consultation to primary care and school staff pertaining to the behavioral health needs and treatment goals of their patients.
- Network/collaborate with community leaders, health care providers, community-based organizations, and others who contribute to the ongoing assessment of the health needs of this group of patients.
- Conduct community outreach to: provide information/make referrals to/coordinate services; identify community resources that provide needed support for patients.
- Shares best practices and expertise with both school and NSCH staff.
- Works in collaboration and under the direction of the school-based site manager (if applicable).
- Meet documentation standards in a timely fashion using the electronic health record for each patient contact.
- Attends in-service training for ongoing licensure requirement and complies with all policies for compliance and documentation.
- Participate in supervision, team and staff meetings, on committees and task forces, as required.
- Enjoys and works effectively in a culture of collaboration, ongoing change, commitment to racial equity, diversity and inclusion.
- Perform additional duties to fulfill NSCH's mission, as assigned.
Requirements
- Current MA LICSW, LMHC, LMFT, or Psy.D licensed required.
- School based health experience preferred.
- Demonstrated strong communication skills with individuals and/or groups.
- Strong analytical, problem solving, planning and prioritization skills.
- Experience preferred of Electronic Health Records and in Microsoft Office.
- Experience using telehealth platforms.
- Self-motivated and self-directed to work within a team and work independently.
- Maintain CEU’s as required by the state.
- Bilingual Spanish/Portuguese preferred.
- Reliable transportation.
