At your first visit

You will register with the receptionist and fill out some paper work. If you need assistance, it will be provided. If you do not have insurance, our enrollment coordinator will assist you in obtaining medical coverage.

Insurance/Health Safety Net (Free Care)

Fees:

We accept most private insurance plans, Medicaid, Medicare, Health Safety Net (FreeCare,) Commonwealth Care (Neighborhood Health Plan & Network Health) and Commonwealth Choice Plans.  Financial counselors are available daily to determine the type of medical fee assistance that is best suited to you.  No one will be denied service because of race, sex, religion, sexual orientation, ethnic orientation, or ability to pay.  If you have any question regarding what health plans are accepted, please call one of our offices.

If you are uninsured you may be eligible for Medicaid, Commonwealth Care Plan, or the Health Safety Net (FreeCare.) One of our Financial Counselors can assist you in applying for medical & dental coverage for you and your family.

Financial Counselors see patients on a walk in, first come first serve basis Monday-Friday between 9-12 and 1-4.

You will need to provide information regarding family size and income in order to process your application for coverage through the State of Massachusetts’ Common Intake Form through the Virtual Gateway. You will also need to bring proof of any family income including paystubs (2 most recent consecutive,) monthly social security statements, recent tax return (for self employed income,) proof of child support/alimony, and proof of any other household income. (Household is defined as an individual, spouse, and any children under the age of 19.)

North Shore Community Health, Inc. Sliding Fee Program

Patients not qualifying for assistance will be given the opportunity to apply for our sliding fee program and/or make a payment plan with our billing department.  Discounts range from a minimum of $20 per visit, or 25%-75% discount on charges depending on family size and income.  To apply for our sliding fee program please complete the sliding fee application and submit with proof of income (1 paystub, tax return, social security statement, etc.)
